REQUEST FOR PROPOSAL TRANSPORT SERVICES

Heifer International Zambia is requesting quotations from registered firms to provide transport services.
The Request for Proposals (RFP) further specifies the specifications of all the information that the firms will need to provide a formal quote as indicated below:
Scope:
Transport services for office furniture, materials, records, appliances, and equipment typically found in a professional setting. All Labor, supervision, tools, equipment, transportation, permits, fees, taxes, incidentals, and materials necessary to perform move are to be included and provided by the contractor.
Locations:
The transport service will take place from our office /in Choma (Southern Province) and the destination shall be Lusaka (Lusaka Province). All materials are to be provided by the contractor (i.e., boxes/bins/crates, carts, packing protection, etc.). All goods are to be wrapped in protective materials to ensure they are protected from any minor or major damage.
The contractor will be required to assemble/disassemble, takedown/setup furniture if needed for transport/moving.
Prospective Bidders are required to submit the following:
A single quotation for a 5-ton containerized truck
Company Profile
Registration/Incorporation Certificate from PACRA or Registrar of Societies.
Evidence of similar past or completed or ongoing delivery works in the last 3 years.
Tax Clearance Certificates from ZRA for the last two (2) years for non-cooperatives.
Terms of payments.
